Minnesota — September 1, 2025 — Vitality Healthcare Management is excited to announce the expansion of its continuum of care with the official launch of our new Home and Community Based Services designed to help Minnesotans remain safe, supported, and independent in their homes.
Beginning September 1, 2025, the new service line will serve the following counties in Minnesota, St. Louis, Stearns, Benton, Aitkin, Cass, Crow Wing, Itasca, and the Seven County Metro. Vitality will offer a variety of essential home care services, including personal care, skilled nursing, medication management, physical, occupational and speech therapy, post-operative care, and companionship.
The expansion was made possible through the acquisition of Spectrum Community Health, an existing home care company. All current staff from the acquired provider will transition to the Vitality team, ensuring continuity of care for existing clients and a smooth operational start. The ownership and strategic leadership will be provided by Tyler Hoemberg, owner of Vitality Healthcare Management and its service lines, which currently includes skilled nursing home, rehabilitation and assisted living services.

This launch represents a key step in Vitality’s broader strategic mission to “own the continuum” of care. By integrating in-home services, Vitality aims to better support individuals through continuity of care, excellent quality and seamless transitions throughout the continuum of care. The same high standards of clinical oversight, convenience, and quality that define other Vitality service lines will apply to the new home care offerings.
